Endovenous Laser Treatment (EVLT) is a minimally invasive procedure used to treat varicose veins of the lower extremities. This treatment uses laser energy to close a functioning vein, usually a superficial vein in the leg.
Endovenous means inside the vein, a small amount of local anesthetic is injected into the skin over the vein and a needle is inserted into it. A wire is passed through the needle and up the vein. The needle is removed and a catheter is passed over the wire, up the vein and the wire is removed. A laser fiber is passed up the catheter so its tip lies at the highest point to be heated (usually your groin crease). A large quantity of local anesthetic solution is then injected around the vein through multiple tiny needle pricks.
The laser is then fired up and pulled down the vein to heat the lining within the vein, damaging it and causing it to collapse, shrink, and eventually disappear
Effect of different lasers on varicose veins
980nm/1470nm/1940nm
980nm laser is mainly absorbed by hemoglobin, suitable for the treatment of thick deep veins (>6mm), but the thermal diffusion is more obvious; 1470nm with water molecules as the main target, suitable for medium veins (3-6mm), thermal damage is smaller, faster recovery; 1940nm due to the strong water absorption, dedicated to the surface of the small veins and capillary dilatation, high precision and virtually non-invasive!
The choice of treatment depends on the diameter and depth of the vein and the patient's skin type: 980 nm is preferred for thick veins, 1470 nm is recommended for medium veins, and 1940 nm for spider veins; combination treatments (e.g., 980 nm+1940 nm) allow for deeper closure and surface modification but require a balance between efficacy and safety, avoiding excessive thermal damage.
Why studies recommend 1470nm diode laser for varicose vein treatment?
If you see the chart the absorption coefficient is higher for hemoglobin than water for 980nm radiation. For 1470 nm the situation is opposite and water absorbs the radiation better than hemoglobin.
Since vein structure is mostly water and 1470nm laser is water specific laser, the laser energy is preferentially absorbed in the intracellular water of the vein wall and in the water content of blood.
Thereby it efficiently heats the vein with little chance of collateral heating, resulting in an optimal vein ablation. The irreversible photo-thermal process induced by the laser energy results in a complete occlusion of the treated vein. Unlike 980nm radiation EVLA which needs appropriate power of 10-12 watt for a treatment, 1470nm radiation needs only 6-8 watt. This low energy level significantly minimizes adverse effects
